每種手術(shù)都適合微創(chuàng )嗎

微創(chuàng )手術(shù)傷口小，病人術(shù)后的復原較快，但每種手術(shù)都適合微創(chuàng )嗎？要開(kāi)刀了，到底要選擇傳統開(kāi)刀方式，還是微創(chuàng )手術(shù)呢？

微創(chuàng )手術(shù)顧名思義是相對于傳統手術(shù)的大傷口而言，對人體所引起的創(chuàng )傷較小，但是，卻可以達到相同目的的一種手術(shù)方式。隨著(zhù)醫學(xué)進(jìn)步、器械發(fā)展日新月異，現在的微創(chuàng )手術(shù)從頭到腳都可執行，算是相當常見(jiàn)的手術(shù)方式。

微創(chuàng )手術(shù)傷口小復元快

臺大醫院新竹分院肝膽腸胃外科主治醫師黃俊杰表示，「微創(chuàng )手術(shù)（ Minimally Invasive Surgery ）」這個(gè)名詞最早在 1970 年代，主要的方式是在體表、體腔或是經(jīng)由人體自然的開(kāi)口，例如尿道、 *** 、 *** 、嘴巴等地方制造幾個(gè)小切口，把內視鏡或類(lèi)似的監視構造以及手術(shù)器械伸到體內，再由醫師自體外透過(guò)電腦和螢幕的監視，操作這些器械來(lái)進(jìn)行手術(shù)。

相對于傳統手術(shù)，微創(chuàng )手術(shù)最主要的好處是傷口比較小，所造成的傷口疼痛跟疤痕也較小，恢復正?；顒?dòng)的時(shí)間比較快，住院天數也較短，術(shù)后沾黏的機率比較低，對醫師而言，也較能看清楚手術(shù)范圍的全貌，達到令人滿(mǎn)意的手術(shù)結果。

但微創(chuàng )手術(shù)也有缺點(diǎn)，包括手術(shù)成本高、手術(shù)技術(shù)性高、手術(shù)時(shí)間相對比較久、醫師學(xué)習曲線(xiàn)相對比較長(cháng)等等，然而隨著(zhù)科技的進(jìn)展、器械的不斷推陳出新，以及手術(shù)醫師的經(jīng)驗累積，這些缺點(diǎn)已經(jīng)逐漸被克服。所以，目前微創(chuàng )手術(shù)已經(jīng)被廣泛運用在許多外科手術(shù)上。

不是每種手術(shù)都適合微創(chuàng )

黃俊杰表示，理論上所有的手術(shù)都有相對比較微創(chuàng )的方式，許多患者希望接受微創(chuàng )手術(shù)，不過(guò)，這樣的方式真的比較好嗎？其實(shí)并不盡然，因為如果只是讓傷口縮小、傷口數目變少，而采用微創(chuàng )的手術(shù)方式，有時(shí)候反而會(huì )增加疼痛感、傷害到體內更多的組織、大幅增加手術(shù)的時(shí)間，或是沒(méi)辦法把癌細胞清干凈，對病患來(lái)說(shuō)傷害反而更大。

微創(chuàng )手術(shù)的真正精神是利用手術(shù)的技術(shù)，以及現有的科技還有器械，幫助病人受到的創(chuàng )傷變小，而且又能達到同樣的手術(shù)目的，不單純只是追求傷口變小變美觀(guān)而已，也不是為了醫師和醫院的利益而作，所以，并不是每項手術(shù)或是每位病人都適合。

廣義的微創(chuàng )手術(shù)包括身體的微創(chuàng )、心理的微創(chuàng )、經(jīng)濟的微創(chuàng )，甚至是對醫師的微創(chuàng )等各個(gè)面向，不單是指傷口或手術(shù)本身，而是需要整體考量，也就是所謂的個(gè)人化醫療。因此黃俊杰建議，每位病人要接受手術(shù)的時(shí)候，應與醫師討論并且了解各種手術(shù)方式的適應癥、優(yōu)缺點(diǎn)、后遺癥，以及所需要的花費和可能發(fā)生的風(fēng)險，以便量身打造一個(gè)最合適的「微創(chuàng )手術(shù)」。

常見(jiàn)的微創(chuàng )手術(shù)種類(lèi)

黃俊杰表示，在體內操作的微創(chuàng )手術(shù)，需要足夠的手術(shù)空間，而腹膜腔（包括腹腔以及骨盆腔）是人體最大的自然空腔，所以，腹腔鏡的手術(shù)是最早，也是最常運用的微創(chuàng )手術(shù)方式。

◎腹腔鏡手術(shù)：從最早期的輸卵管切除、結扎、闌尾切除手術(shù)開(kāi)始，隨著(zhù)器械的改良，大量運用于膽囊切除術(shù)、胃腸道功能性手術(shù)以及疝氣手術(shù)。在許多內視鏡自動(dòng)器械包括諧波刀（超音波手術(shù)刀）、組織凝集器以及胃腸自動(dòng)吻合器出現之后，腹腔鏡手術(shù)更廣泛運用于腹腔內各個(gè)器官，包括胃腸、脾臟、胰臟、肝臟、膽管、大腸直腸、腎上腺、腎臟、輸卵管、膀胱、卵巢、子宮等部位的手術(shù)。隨著(zhù)時(shí)間的累積，已經(jīng)有許多文獻證實(shí)：腹腔鏡手術(shù)和傳統手術(shù)相比，對于部分癌癥的預后有相同的效果，也因此腹腔鏡手術(shù)更加蓬勃發(fā)展，并且進(jìn)展到傷口 2 公分左右的單孔腹腔鏡手術(shù)，以及最新的機器手臂輔助手術(shù)等。

◎胸腔鏡手術(shù)：在腹腔之外，胸腔是另一個(gè)人體比較大的自然空腔，目前胸腔鏡手術(shù)的發(fā)展也已經(jīng)成熟，大幅減少需要開(kāi)胸腔手術(shù)病人的不適。

◎其他部位手術(shù)：除了以上兩者之外，人體許多部位的微創(chuàng )手術(shù)也在不斷的發(fā)展中，幾乎可以說(shuō)「從頭到腳」都能?chē)L試，包括某些顱內病灶、鼻竇炎、甲狀腺、 *** 、四肢關(guān)節、心臟（冠狀動(dòng)脈繞道手術(shù)）、脊椎（椎間盤(pán)突出）、腳部靜脈曲張的手術(shù)，使得患者受到的傷害越來(lái)越小，也能維持傷口的美觀(guān)。

該選擇傳統手術(shù)或微創(chuàng )手術(shù)？

《 視病況而定 》

黃俊杰表示，目前某些腹腔鏡手術(shù)已非常成熟，例如膽囊切除和闌尾切除，幾乎都是用腹腔鏡執行，不過(guò)，有部分的腹腔手術(shù)仍要視情況而定，并不完全適合微創(chuàng )。

以胃癌而言，目前技術(shù)上是可以做到微創(chuàng )，也確實(shí)可以達到傷口小、恢復快速的目的，但是，對于較晚期或是淋巴結轉移病患的長(cháng)期預后，仍缺乏文獻證明與開(kāi)腹手術(shù)有相同的效果。癌癥手術(shù)的目的是為了把淋巴結和癌細胞清干凈，胃的某些淋巴結要花較多時(shí)間去清，腹腔鏡手術(shù)不見(jiàn)得可以把癌細胞清得更干凈，因此，晚期胃癌腹腔鏡手術(shù)的成效，仍有待文獻和時(shí)間來(lái)證明。

但同樣是腹腔癌癥，大腸癌就沒(méi)這個(gè)問(wèn)題，論文證實(shí)傳統與微創(chuàng )手術(shù)的效果相當，而且如果是直腸癌，因為位于骨盆腔深處，傳統開(kāi)法比較難看清楚，用腹腔鏡或機器手臂手術(shù)反而比較看得到病灶，因此，目前已被大多數醫師采用。另外，同樣是惡性腫瘤，胃腸道基質(zhì)瘤因為不需要清淋巴結，也比較適合腹腔鏡手術(shù)。

許多醫院紛紛引進(jìn)的機器手臂手術(shù)，因為機械手臂穩定度比較高，手術(shù)過(guò)程不會(huì )抖，機械手臂的關(guān)節可以 360 度旋轉，靈活度更高，在某些空間狹窄的地方更適合做，比如甲狀腺手術(shù)、泌尿科的攝護腺手術(shù)，需要精細的動(dòng)作時(shí)，可以發(fā)揮更大的效益。

《 視病人身體狀況而定 》

有些手術(shù)，因為技術(shù)性和復雜度較高，開(kāi)刀時(shí)間原本就需要較久，例如胰頭癌的主要手術(shù)方式是「胰頭十二指腸切除術(shù)」，切除范圍包括胰頭、十二指腸、空腸上端、胃竇部、膽囊、總膽管等，一臺刀需要 4 ～ 10 小時(shí)不等，如果改用腹腔鏡方式進(jìn)行，可能要操作更久的時(shí)間，對病人傷害可能會(huì )更大，因此雖然技術(shù)上可行，但只有少數醫師采用。

另外，因為**時(shí)間長(cháng)，老人或血壓不穩的人，開(kāi)完那么久的刀，反而有可能并發(fā)中風(fēng)或心肌梗塞等嚴重后遺癥，因此，要評估病人的身體狀況，如果是老人家或身體虛弱、免疫力差的人，不見(jiàn)得適合采取微創(chuàng )方式進(jìn)行手術(shù)。

此外，若是最近腹腔內開(kāi)過(guò)刀，尤其上下腹部都開(kāi)過(guò)的，因為腹內組織沾黏，就不適合用腹腔鏡。但若上腹部開(kāi)過(guò)刀，下腹部還有地方開(kāi)洞，還是可以考慮的；或是 10 年前開(kāi)過(guò)刀，可能沾黏沒(méi)那么厲害，也可以考慮使用腹腔鏡來(lái)手術(shù)。黃俊杰表示，手術(shù)前任何檢查都看不出組織沾黏的程度，所以病人要先告知，讓醫師了解詳情，再評估適合哪一種手術(shù)方式。

《 考量經(jīng)濟問(wèn)題 》

有些微創(chuàng )手術(shù)健保并沒(méi)有給付，或即便健保給付手術(shù)費用，但某些材料并不給付，因此病人在選擇手術(shù)方式前，可以先詢(xún)問(wèn)清楚，以免身體的傷口小，但經(jīng)濟的缺口卻變大了。

微創(chuàng )手術(shù)有健保給付嗎？

黃俊杰表示，微創(chuàng )手術(shù)的類(lèi)別很多，通常健保都有給付手術(shù)費用，但某些材料不給付，例如諧波刀、組織凝集器、胃腸自動(dòng)縫合**等。健保有給付部分胃腸縫合釘的費用，但比較不會(huì )滲漏和流血的高階縫合釘則要自費，一釘約 1 萬(wàn) 5000 元左右。

因此，以腹腔鏡的胃部手術(shù)來(lái)說(shuō)，若全部都要用高階縫合釘加上一只縫合**得花十幾萬(wàn)元，并不是每位病人都可以負擔的，有的病人寧可傷口大一點(diǎn)或只用健保釘，就可以來(lái)省下好幾萬(wàn)元。
