AAH階段不必要手術(shù)。AIS、MⅠA為最早期肺癌，完全切除或局部切除即可達幾乎100%的治愈率。

2011年2月，國際肺癌研究協(xié)會(huì )(IASLC)、美國胸科協(xié)會(huì )(ATS)及歐洲呼吸學(xué)會(huì )(ERS)在《Journal of thoracic oncology》上聯(lián)合發(fā)布了肺腺癌國際多學(xué)科分類(lèi)新標準，即新版肺腺癌 IASLC/ATS/ERS分類(lèi)：

(1) 不典型腺瘤樣增生(atypical adenomatous hyperplasia, AAH)：肺末梢組織局灶性增生，≤5mm，不典型的立方或柱狀上皮細胞代替原來(lái)的正常肺泡上皮細胞，沿肺泡或呼吸細支氣管分布，是AIS的癌前病變。

(2)原位腺癌(adenocarcinoma in situ, AIS)：≤3cm，原版細支氣管肺泡癌(bronchiolo-alveolar carcinoma, BAC)，分非粘液型、粘液型、混合型。

2. 微浸潤腺癌(minimally invasive adenocarcinoma, MIA)：

≤3cm，鱗屑生長(cháng)方式，浸潤≤5mm，分非粘液型、粘液型、混合型。

3. 浸潤性腺癌(invasive adenocarcinoma, IAC)：鱗屑樣為主型(lepidic predominant, LPA，為IAC 主要類(lèi)型，鱗屑生長(cháng)方式，浸潤>5mm，原版非粘液型BAC)、腺泡為主型、乳頭為主型、微乳頭為主型、伴粘液的實(shí)體為主型(以實(shí)性生長(cháng)方式為主，伴粘蛋白產(chǎn)生)。

4. 浸潤性腺癌變異型(invasive adenocarcinoma variation, IAV)：浸潤性粘液型腺癌(invassive mucous adenocarcinoma, IMA，原版粘液型BAC)，分膠質(zhì)型、胚胎型、腸型。
