按照天主教會(huì )的傳統說(shuō)法，耶穌基督的第一個(gè)門(mén)徒彼得乃眾門(mén)徒之首，他于傳教過(guò)程中去羅馬擔任了羅馬教會(huì )的第一任主教。從此，羅馬主教均為伯多祿的繼位人，其地位因而也在其他主教之上。這便是“教皇制”的由來(lái)。所以，“教皇”的全稱(chēng)為“羅馬教區主教、羅馬教省都主教、西部宗主教；梵蒂岡君主、教皇”，亦稱(chēng)“宗徒彼得的繼位人”、 “基督在世的代表”等。

在天主教會(huì )的教階體制中，教皇享有最高的立法權和司法權，能制定或廢除教會(huì )法規，指定人員組成教廷，創(chuàng )立教區，任命主教，而且“在倫理和信仰上永無(wú)謬誤”。在11世紀前，教皇須經(jīng)世俗君主或意大利貴族遴選或認可。尼古拉二世登基后于1059年決定教皇由樞機主教選舉產(chǎn)生，但須得到法蘭克王亨利一世及其繼位者的認可。直到1179年第三次拉特蘭會(huì )議和1274年第二次里昂會(huì )議兩次確認后，才正式規定教皇可單由樞機主教選舉產(chǎn)生，不過(guò)仍須承認法、西、奧三國君主對候選人具有否決權。20世紀初，庇護十世廢除了這種世俗君主的否決權。1914年，本篤十五世遂成為單由樞機主教選為教皇的第一人。教皇當選后任職終身，不受罷免，但可自行辭職。
